We currently offer “Extended Hours” surgeries on a Monday at Central Surgery and Thorpe Surgery alternative weeks: 6:30pm to 7:50pm. If there is a Bank Holiday Monday, in the week, or, we are closed for “Time to Learn”. (Training sessions for Doctors and Staff are held up to ten times a year, normally on the first Tuesday in the month) the “Extended Hours” surgeries will be held on either Tuesdays or Thursdays at Central Surgery when there is a Bank Holiday Monday. Please contact Central or Thorpe Surgery to book appointments.
We conduct a Wart Clinic once a month; Patients need to see a Doctor before booking an appointment.
Please book your appointments for the Doctors, Practice Nurse or Healthcare Assistant’s (HCA) clinics at the reception or by telephoning the surgery. Each appointment is for one person only.
If you feel that you have an urgent appointment, please inform the Receptionist and you may be either added on, to the current surgery or be offered a “Telephone Consultation appointment”.
Please cancel any appointment in good time, if you cannot attend as this, can then be offered, to another Patient.

New Patients wishing to register with our Practice
The areas listed below are our two separate catchment areas. Please bring your NHS Medical Card or ID with you to register with our Practice using a GMS1 registration form. Please note that you will have to give details of your previous GP and your previous address. It would be ideal if you know your NHS Number. The Receptionists will also give you forms regarding data sharing and whether you wish to register or opt out of a SCR (Summary Care Record). If you are registering children there are additional forms to complete. We invite all new Patients aged over 5yrs old to have a New Patient Check with our Healthcare Assistant (HCA) or our Practice Nurse. Once registered with our Practice, you may book an appointment with any of the Doctors.
All Patients have a Named Accountable GP allocated to them; currently this is Dr Kumar. Please ask the Receptionist for details of this in case of an update.

Patient Participation Group
Patient Participation Group meetings are held by the Practice on a two monthly basis. All Patients are welcome to join; please ask the Receptionist for details to join.
Community Nurse (District Nurse)
The Community Nurse visits the housebound if this is absolutely necessary and you cannot attend at the surgery.

Please contact the surgery to get results of blood test and x-rays etc. Please bring specimens for the hospital to the surgery before 12:00 midday Monday to Friday.
Whilst we make every effort to provide you with the highest standard of care we appreciate that you may feel less than satisfied at times. If you have a complaint, in the first instance please discuss it with a member of staff or, if you prefer, the Practice Manager. You may be asked to put your complaint in writing, which we will respond to and investigate. All complaints will be dealt with without any discrimination by our Practice. Please ask the Receptionist to read a copy of our Complaints Procedure.

We operate a zero tolerance policy towards violent, threatening and abusive behaviour.
Home Visits requests
Please contact the surgery to make home visit requests before 10:00am. Home visits requests are for the housebound only or Patients who are too ill to attend Surgery.
There are car parking facilities at both surgeries and we have wheelchair access.
Freedom of Information Act
The Practice will consent to requests under The Freedom of Information Act 2000.
Requesting repeat prescriptions
No repeat prescription requests over the telephone please - written requests only (this is to avoid any mistakes occurring).
Requests for repeat prescriptions may be handed in at reception. Please use the counterfoil (white) section which is attached to your prescription whenever possible. Please include a stamped, addressed envelope if you wish your prescription to be posted to you. Please allow 48 hours for all repeat prescriptions before collecting them. You can make arrangements with local pharmacies to collect your prescriptions on your behalf or you can register for the EPS (Electronic Prescription Service) in order to elect to send your prescriptions electronically to a particular pharmacy. Please speak to the receptionist at the surgery to arrange this.
You can also register Online to request your prescriptions, make appointments and request to see your medical records. To arrange this, please bring ID with you. eg Passport, Driving Licence etc and speak to the Receptionists to register for this “On-line” service.
Alternatively, the Receptionist may register you for Online services if you are known to them but this does not apply to newly registered Patients.
There is a list in reception areas of the list of private fees for non NHS work undertaken by the Doctors.
e.g. HGV examination medicals, private certificates etc.
NHS Certificates (Med 3) are given by the Doctors after one week of sickness from work. Your employer should give you SC2 forms (Self Certification Forms) to complete for any sickness period for up to a week. If you require a private certificate for up to the first week of sickness, there is a fee. If you are in hospital, then the ward can provide you with a certificate.
All information is recorded on the computer and we are registered under the Data Protection Act. The practice will ensure that patient confidentiality is maintained at all times by all members of the practice team. However, for the effective functioning of a multi-disciplinary team it is sometimes necessary that medical information about you is shared between members of the team. The Practice works to Information Governance guidelines.
