There is help in Southend for patients with Dementia and their family and carers.
There are several Community Services to enable patients to maintain independence, have choice and control over their life and live well with dementia.
As soon as Patients are diagnosed with a form of Dementia there is support from the Community Services.

Jessica is holding a HUB at the surgery every other Monday between 9.00am and 11.00am starting 28 January 2019 to advise our surgery patients who need help or know of someone who needs help. Please come along if you have any concerns.

I can provide support to you and your family throughout the dementia experience, enabling you to maintain independence, have choice and control over your life and live well with dementia.
I can offer support and guidance through community dementia services from pre-diagnosis of dementia through to end of life care.
I am your link between the support you need and health, social care and community services.
